Care Coordinator

Job Summary: We are seeking a compassionate and organised Care Coordinator to join our home care business and assist clients in receiving the highest quality of care. As a Care Coordinator, you will play a crucial role in assessing client needs, developing care plans, coordinating services, and ensuring the well-being of our clients. Your exceptional communication skills and ability to establish rapport with clients and their families will be key in providing personalized care and maintaining strong relationships.

Responsibilities:
  • Conduct comprehensive assessments of clients’ needs. Additionally, including physical, emotional, and social aspects, through interviews and evaluations.
  • Collaborate with clients, their families, and healthcare professionals to develop personalized care plans that meet individual needs and preferences.
  • Coordinate and schedule care services, including home health aides. Fundamentally includes nursing visits, therapies, and other necessary services, ensuring timely delivery of care.
  • Communicate effectively with clients, their families, and caregivers. Thereby ensuring a clear understanding of care plans, and any changes in the client’s condition or needs.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure optimal client outcomes.
  • Provide guidance and support to caregivers, ensuring they have the necessary information and resources to deliver quality care.
  • Maintain accurate and up-to-date client records. Incorporating assessments, care plans, progress notes, and any other relevant documentation..
  • Stay current with industry trends, best practices, and regulations related to home care services, and ensure compliance within the organisation.
  • Serve as a liaison between clients, their families, and healthcare professionals.
  • Participate in care conferences and meetings to share updates.
  • Collaborate with the interdisciplinary team, and contribute to the continuous improvement of care delivery.
Qualifications:
  • Qualifications concurrent with a senior role in a healthcare-related field (e.g., Nursing, Social Work, Senior Carer) preferred.
  • Proven experience as a care coordinator or similar role in a healthcare setting, preferably in-home care.
  • Strong knowledge of home care services, care planning, and community resources.
  • Excellent communication and interpersonal skills to establish rapport with clients, families, and healthcare professionals.
  • Ability to assess client needs and develop individualized care plans.
  • Strong organisational and time management skills to coordinate multiple client care schedules effectively.
  • Empathy, compassion, and patience in dealing with clients and their families.
  • Proficient computer skills and familiarity with electronic health records (EHR) systems.
  • Knowledge of relevant legal and regulatory requirements governing home care services.
  • Ability to work independently, prioritise tasks, and adapt to changing situations.
  • Valid driver’s license and reliable transportation.
